GLPG-0492, an orally available selective androgen receptor modulator (SARM) was tested in a Phase I Proof of Mechanism study to assess the effect on muscle function in healthy volunteers. A biomarker effect similar to that of Oxandrolone was observed, but the data were insufficient for Galapagos to pursue GLPG-0492 further in cachexia, and further development of the compound was discontinued. GLPG-0492 is currently under development for musculo-skeletal diseases such as sarcopenia and Duchenne muscular dystrophy.

Camptothecin-20(S)-O-propionate (CZ48) is a prodrug of camptothecin, an alkaloid isolated from the Chinese tree Camptotheca acuminata, with potential antineoplastic activity. Upon entry into cells, camptothecin-20(S)-O-propionate is hydrolyzed by esterases into the active form camptothecin. Camptothecin selectively stabilizes topoisomerase I-DNA covalent complexes, thereby inhibiting religation of topoisomerase I-mediated single-strand DNA breaks and producing potentially lethal double-strand DNA breaks when encountered by the DNA replication machinery. CZ48 is being investigated in clinical trials for the treatment of advanced solid tumors and lymphomas.

D-aspartic acid is an essential amino acid and a key ingredient in various testosterone boosting anti-estrogen supplements. D-aspartic acid is not used to build proteins; instead, it plays a role in making and releasing hormones in the body. It is an endogenous NMDA receptor agonist with similar activity to the L-isomer. D-aspartic acid also enhances the release of luteinizing hormone (LH) and testosterone. This action is mediated in the pituitary by cGMP and in the testis by cAMP, which acts as the second messengers in the signal transduction in the pituitary and testes respectively. The pituitary and testis possess a D-Aspartate racemase, which provides the necessary production of this isomer.

Orteronel (TAK-700) is a non-steroidal antiandrogen, discovered by Takeda Pharmaceutical Company Limited, that selectively inhibits the 17,20 lyase enzyme (CYP17A1). This enzyme, which is present in both the testes and adrenal glands, is central to the production of steroidal androgens. Synthesis of androgens outside the testes contributes to disease progression in castration-resistant prostate cancer (CRPC). In phase III of clinical trials for metastatic, hormone-refractory prostate cancer it wasn’t shown overall survival rates, and development was voluntarily terminated as a result.

Prodilidine is an arylpyrrolidine derivative patented by Mead Johnson & Co.as moderately potent analgesic. By comparison with other drugs, including morphine, Prodilidine has a prompt and unusually sustained antinociceptive action in rodents by all routes. Prodilidine substantially lacks antipyretic, anti-inflammatory, antitussive, constipating, respiratory depressant, and cardiovascular effects. Prodilidine resembles meperidine in excitatory properties and body-temperature lowering action at high dosage. The d-isomer is about twice as potent and toxic as the l-isomer parenterally; by the gastric route, the difference is markedly less.
