Norepinephrine hydrochloride, (±)- (DL-Norepinephrine Hydrochloride) is a vasoconstrictor, cardiac stimulant, α- and β-sympathomimetic agent. DL-Norepinephrine hydrochloride is an adrenergic drug. Norepinephrine is an endogenous catecholamine that is the neurotransmitter at sympathetic postganglionic fibers. It has potent beta1- and alpha-stimulating effects. In contrast to epinephrine, norepinephrine has only minor effects on beta2 receptors. The clinical effects of norepinephrine administration are mainly increased cardiac index and increased vascular (systemic and pulmonary) resistance. Several adult studies have suggested that norepinephrine is useful in increasing SVR in patients with hyperdynamic or vasodilatory septic shock that is not responsive to dopamine or epinephrine. Additionally, it can augment coronary blood flow by increasing systemic diastolic pressure, at the expense of increasing afterload.

Levisoprenaline (l-isoproterenol) is beta1- and beta2-adrenoreceptor agonist. In Japan, continuous inhalation of levisoprenaline is a recommended treatment for pediatric patients with acute severe exacerbation. Compared with salbutamol, l-isoproterenol reduced modified pulmonary index score more rapidly. But in most countries do not recommend continuous inhalation of l-isoproterenol to treat pediatric patients with acute severe exacerbation of asthma due to worsening asthma control. Levisoprenaline was used to cause experimental chronic heart failure in rats.

Nifenalol is the beta-receptor antagonist. It has optical isomers. The racemic mixture and the levo-isomer are active in antagonizing beta-receptors, but the dextro-isomer is inactive. The levo-isomer seems to be about twice as active in blocking beta-receptors as the racemate. Nifenalol is virtually devoid of local anesthetic properties in contrast to procaine, propranolol, and butidrine. Nifenalol exacerbated the fighting behavior in male mice by foot-shock. Nifenalol has been studied in patients with coronary artery disease. It afforded the coronary patient good protection against angina and ischemic changes in the EKG. It was further noted that nifenalol had no antiarrhythmic action and that it was devoid of evident side effects. Nifenalol possessed weak action against tremorine and oxotremorine induced tremor.

Picumeterol (GR114297A) is a potent and highly selective beta2-adrenoceptor agonist with a longer duration of action than salbutamol, but shorter than salmeterol. Picumeterol is the (R)-isomer of the racemic entity GR63411B, and picumeterol has been shown to be about 40 times more potent than the (S)-isomer (GR114744A) in various in vitro pharmacological models of beta2-agonist activity. Picumeterol is of potential value in the treatment of asthma and related diseases in man following inhalation administration. In the clinical studies, the bronchodilator potencies of picumeterol and GR 63411B were similar. However, both drugs were short-acting, which is at odds with their activity in vitro. These compounds display dissociation between bronchodilator activity and protection against methacholine-induced bronchoconstriction.
